Completing your Certificate Application
This form should be completed for students who have completed the birth to three specialization and wish to complete their certificate.

Please attach a current résumé indicating work experience, education, and other interests and activities. Document should be in PDF format.
In a PDF document:
· What work (past and present) with young children have you taken up since your completion of your Birth to Three Specialization? (where, size of group, ages of children, your role)
· What further studies have you undertaken? Include any longer term studies in progress. These may be formal or informal studies.
· What are you reasons for interest in completing your certificate?
· With what questions are you presently living in regard to your vocation as a Waldorf early childhood teacher?
AGREEMENT
It is expected that each course participant will be actively engaged with young children as a caregiver or early childhood teacher in a Waldorf setting.
Students must have technological ability, own or have access to a computer, and have access to an email account. Some coursework requires use of a computer, so students must have basic word processing ability to be able to complete all assignments. Communication between the Institute and program students generally takes place via email.

I understand that photos and video may be taken of me while in the program. I understand that is my responsibility to notify Sophia’s Hearth in writing if pictures of me should not be used for marketing materials.
I understand that falsifying any part of this application may result in cancellation of admission and/or enrollment.
